SIDE 1 - A red disc surrounded by a dark blue band with gold borders, with a crossed gold quill detailed in red and a gold micrometer caliper outlined in yellow, the bottom of both piercing the band, with the word, "FORWARD" at the top of the band, and "WITH SUPPORT" at the bottom in gold letters.

SIDE 2- A red disc with the detail of a map of Korea in gold at center, US and Korean flags at the top, and "194th Combat Sustainment Support Battalion 'Provides'" directly below, a Sergeant Major's insignia of rank labeled "Provider 7" to the left of the map and a Lieutenant Colonel's insignia of rank labeled "Provider 6" to the right; surrounded by a blue band with bulges where the quill and caliper on the other side extend beyond the band, the words, "Presented for Excellence" at the top, "Team Provider

at the bottom in gold letters and red 5-pointed stars to the left and right.
